Job summary

Essex Partnership NHS Foundation Trust is seeking a Crisis Resolution Home Treatment practitioner to support individuals in the community, working in partnership with families and carers.

You will maintain confidential communications across health and social care sectors, assess risks, and participate in on-call arrangements to ensure continuous care. A strong commitment to professional standards and inter-agency collaboration is essential.

Responsibilities

  • Be aware of own values, beliefs and attitudes when assessing clients to ensure a non-judgmental, objective overview.
  • Establish a therapeutic relationship with individual clients and their families.
  • Ensure effective and confidential communication between health and social care agencies on significant data governing client care and treatment by participating in case conferences, maintaining secure records, and arranging regular reviews.
  • Be aware of clinical risk assessment and the ongoing monitoring of a client’s mental state to evaluate risk to themselves or others.
  • Identify clients with behavioural problems in the community and exercise caution during home visits when necessary.
  • Develop close links with relatives/carers and ensure effective communication during hospital admission or discharge, informing appropriate agencies for follow-up care.
  • Advise Line Manager and Supervisors on breaches of professional conduct as required by relevant codes of conduct.
  • Liaise with Line Manager to ensure NHS policies are communicated and implemented within the area.
  • Work flexibly across the Crisis Resolution Home Treatment Service within the Trust.
  • Take on clinical work commensurate with experience in liaison with supervisors and senior staff.
  • Collaborate with colleagues to improve the quality of services and provide on-call coverage for Crisis Resolution Home Treatment as needed.

Job description

  1. 4. To be aware of ones own values, beliefs and attitudes when assessing clients to ensure that a non-judgmental and objective overview of the clients needs are met at all times.
  2. 5. Need to establish a therapeutic relationship with individual clients and their families.
  3. 6. Ensure effective and confidential communication between health and social care agencies on significant data governing client care and treatment by: Participating in informal or formal case conferences and discussions. Maintaining a secure system of record keeping and information exchange. Establishing regular written reviews in progress.
  4. 7. To be fully aware of the importance of effective risk management, the undertaking of clinical risk assessment and the on-going monitoring of the clients mental state to evaluate risk either to themselves or towards others.
  5. 8. To ensure those clients in the community with behavioural problems e.g. violent or potentially violent clients are identified in caseload and that caution and care is taken when visiting such clients, if necessary organising joint visits.
  6. 9. To develop close links with relatives/carers and ensure that effective communication takes place especially when clients are admitted to hospital or preparing to be discharged and that all appropriate agencies are informed so that follow-up care can take place.
  7. 10. To advise Line Manager and Professional Social Work Supervisor and/or Nurse Advisor on all breaches of Nursing/Social Work/Occupational Therapy professional conduct as required by the NMCs/HCPC Code of Professional Conduct.
  8. 11. To liaise with Line Manager to ensure that all Essex Partnership NHS Foundation Trust Policies and Procedures are communicated, implemented and developed within the area.
  9. 12. To work flexibly across the whole spectrum of Crisis Resolution Home Treatment Service within Essex Partnership NHS Foundation Trust.
  10. 13. To take on clinical work commensurate with experience/skills, in liaison with their supervisor and other senior staff within the team.
  11. 14. To be responsible for working with your colleagues in seeking to improve the quality of services provided to our clients/patients. This will include providing cover arrangements for Home Treatment and Crisis Services at needs of the service within Essex Partnership NHS Foundation Trust.
  12. 15. To participate in the on call arrangements for the Crisis Resolution Home Treatment Service.
