Job summary

Norfolk Community Health and Care NHS Trust is seeking healthcare professionals to assess, prescribe and deliver care packages for patients with long term conditions, palliative care and rehabilitation needs to improve quality of life and independence wherever possible.

You will work within an integrated team to facilitate early discharge from hospital and prevent unnecessary admissions, coordinating with health care professionals and agencies to provide a seamless service to service users.

Job description
