Job summary

Person-centred Support is at the heart of Lighthouse Community Care (LCC) service alongside continued compliance with relevant legislations and providing a high-quality service enabling individual needs.

The Support Worker is responsible for providing high-quality, person-centred support to adults aged 18–65 and 65+, including individuals with mental health needs, dementia, physical disabilities and sensory impairments.

The role focuses on supporting each service user to live as independently, safely and meaningfully as possible, while promoting their choice, dignity, privacy, rights and wellbeing. Responsibilities include supporting daily living activities, medication where authorised, meal preparation, household tasks, community access and social inclusion. Support Workers are also responsible for maintaining accurate records, following individual support plans, identifying and reporting changes or concerns, safeguarding service users and working effectively with families, colleagues and other professionals. All support must be delivered in accordance with Lighthouse Community Care's policies, procedures and relevant regulatory requirements.
