Job summary

A community health service provider in the UK is seeking a supervisor for the Rehabilitation Support Workers team. Job description

This role will be part of the existing Reablement At Home Support team and will enhance the leadership by offering on the job support to our amazing Rehabilitation Support Workers. This service provides short term therapy to promote independence to enable people to remain at home or return home at the earliest opportunity. The role will include supervision, support with training including Holistic Worker competencies, quality of care and patient flow.

  • Support management in line managing RSWs.
  • Support management in making sure RSWs are up to date with training
  • To ensure RSWs have a robust induction and competency training.
  • Identify any further training needs for Integrated Reablement At Home Support Team
  • Supervising RSWs and feeding back to management.
  • Supporting management in data quality and notes auditing
  • Attending daily MDTs and being the link person between clinicians and RSWs.
  • Carrying out face to face patient visits according to service needs.
  • To act as a positive role model at all times contributing to the smooth running of services and to the overall benefit of patients and their carers and the good reputation of the Nottingham CityCare.
  • To maintain a focus on continuity of care and the patient experience at all times.

We are looking to recruit an individual with a minimum of 2 years experience in Health and Social Care and who has achieved a relevant level 4 qualification or above.
