Reablement focuses on promoting independence by supporting people to do as much as possible for themselves both physically and psychologically. We do this by helping customers to continue living in their own home, preventing admissions into hospital or residential settings, and facilitating timely discharges home. We believe that by identifying and achieving personal goals and embedding personalisation, customers will have choice and control over how their support is delivered, which positively impacts health and well-being.
As a Co-ordinator, your role will include line management of Warwickshire County Council’s Reablement Assistants. You will ensure a safe and effective Reablement service is offered to the people of Warwickshire. You will work in partnership with the Reablement Assessment Team and other professionals to provide an outcome‑focused service that promotes independence and maximises daily living skills. You will undertake, develop and manage work programmes for Reablement Assistants to provide a consistent service that is safe, high quality and cost effective. You will manage the Reablement referral through the adult journey within Reablement and ensure that needs and outcomes identified within the Reablement plan are met.
The posts will be Countywide (two based at Kings House, Bedworth and one based at Heathcote House). The post will be for 37 hours per week. There is an expectation that the post holder will provide countywide support to Warwickshire Reablement service, therefore travel across the county is essential.
The Fluency Duty is outlined in the Code of Practice on English language requirements for public sector workers. The Fluency Duty for this role is Required. You must be able to speak and provide advice in accurate spoken English to the public.
