Reablement Assistant Practitioner

Permanent contract | £15,768 to £16,849 per annum (Scale G) | 18.5 hours per week | County Hall, Norwich

Overview

The Caring for Better Outcomes team, aligned with Norfolk First Response, delivers reablement services to individuals sourced into home care. The role supports those newly discharged from hospital or community referrals requiring a 6‑week pathway to maximise independence. You will work within a multidisciplinary team, alongside Assistant Practitioners, Occupational Therapists and a Practice Consultant.

Responsibilities
  • Assess and review individuals for care and support, adopting a strength‑based approach.
  • Coordinate and arrange appropriate services where required.
  • Collaborate with the MDT to develop and implement plans for independence.
  • Attend and contribute to MDT meetings and supervision sessions.
Qualifications & Skills
  • Ability to work under pressure, organised and able to prioritise tasks.
  • Positive “Home First” ethos and commitment to excellent customer service.
  • Excellent communication skills, record‑keeping and keyboard proficiency.
  • Confidence in strengthening social care practice.
  • Full driving licence and ability to drive is essential.
