Job summary

Joined Up Care Derbyshire is seeking a Senior Community Nurse to join the Rapid Response Nursing Team in Derby City. You will deliver acute care in patients\' homes and residential care settings, with 7 days a week coverage from 08:00 to 18:30, focusing on health education and patient self-management.

You will triage and assess, mentor staff, provide high-quality care for complex cases, and participate in service improvement within a multidisciplinary team.

Job summary

We have a fantastic opportunity for a nurse who has experience of community nursing to join our Rapid Response Nursing Team in Derby City as a Senior Community Nurse. Our service provides acute care in patients' own homes and Residential Care Home settings. We promote a self-management approach to patients with their health needs, therefore a key priority is to offer health education at each visit. Our service provides cover 7 days per week, 08:00-18:30 to support patients with urgent nursing needs. You will have the opportunity to develop specialist nursing assessment skills to enable them to provide a comprehensive assessment to a variety of complex patients both via telephone triage and clinical patient home visits. You will be well supported in the role by a skilled multi-disciplinary team and have the opportunity to participate within an in-service training programme. The requirements to travel across Derby City and flexibility are essential for this post.

Main duties of the job
  • Responsible for all aspects of band 6 role including:
  • Triaging, assessing patient priority and allocating visits in accordance with priority and ensuring we meet Urgent Community Response times;
  • Supporting new starters/junior staff/ student nurses with achieving clinical competencies/confidence.
  • Deliver high quality clinical care and providing support with complex patient management and co-ordination of care
  • Support with quality always, audits and data analysis.
  • Staff leadership including; probationary meetings, 1-1's and appraisals
