Qualifications

• Experience in core Physiotherapy rotations is essential.
• Confident and competent in discharge planning.

Responsibilities

• Support the Rapid Response Team therapy caseload.
• Assess and discharge patients in SDEC and A&E.
• Complete discharge visits within 2 hours of hospital discharge.
