Job Summary

Rapid Response Assistant Practitioner / HCA / Hybrid worker. We are seeking a motivated and dynamic Band 4 HCA or Hybrid worker, with an interest in general physical health, for this exciting opportunity to join the Waltham Forest Rapid Response team.

The Rapid Response team is a forward‑thinking, community‑based, fully integrated service that works alongside nurses, physiotherapists, occupational therapists, social workers, community psychiatric nurses and health‑care support to provide an integrated, holistic approach to the care of people accessing our service. The team is expanding in response to the national Urgent Care standards, and this role offers the chance to support the population of Waltham Forest, providing urgent multidisciplinary (MDT) response within 0‑2 hours to prevent avoidable hospital admissions.

Working across seven days a week, 24 hours a day, this role requires flexible shift work and uses your own car – mileage and parking are claimable.

Responsibilities
  • Carry out clinical visits, including falls pick‑up, to patients in their own homes, residential and care homes under the direction of nurses/therapists.
  • Deliver a variety of nursing and therapy skills within the patient’s home environment.
  • Work as part of the multidisciplinary team to support and treat suitable patients with a range of escalated health and social care needs.
Qualifications
  • Minimum two years’ experience as a rehab assistant/practitioner or as HCA/Nurse Associate.
  • Established and competent extended practice at Band 3.
  • Desire to learn and support new ways of working.
  • Good team worker, flexible and able to work under pressure.
  • Excellent interpersonal and IT skills and able to communicate effectively with patients, carers and other MDT members.
  • Assisted practitioner or nurse associate with proven clinical and communication skills are also invited to apply; previous community NHS experience is desirable but not essential.

Salary and Working Details

Band 4 – £27,485 to £30,162 per year, plus HCAS.

High‑Cost Area Supplement: 15% of the basic salary (minimum £4,313, maximum £5,436, pro‑rata for part‑time staff).

Permanent, Full‑time.

Working pattern: Full‑time, flexible shifts across 7 days a week, 24 hours a day.

Probationary period applies. Internal applicants are exempt from the probationary period.
