A community-based health service in London is seeking a motivated Rapid Response Assistant Practitioner to join their multidisciplinary team. This role involves providing urgent healthcare in patients' homes to prevent avoidable hospital admissions. Rapid Response Assistant Practitioner / HCA / Hybrid worker. We are seeking a motivated and dynamic Band 4 HCA or Hybrid worker, with an interest in general physical health, for this exciting opportunity to join the Waltham Forest Rapid Response team.
The Rapid Response team is a forward‑thinking, community‑based, fully integrated service that works alongside nurses, physiotherapists, occupational therapists, social workers, community psychiatric nurses and health‑care support to provide an integrated, holistic approach to the care of people accessing our service. The team is expanding in response to the national Urgent Care standards, and this role offers the chance to support the population of Waltham Forest, providing urgent multidisciplinary (MDT) response within 0‑2 hours to prevent avoidable hospital admissions.
Working across seven days a week, 24 hours a day, this role requires flexible shift work and uses your own car – mileage and parking are claimable.
Band 4 – £27,485 to £30,162 per year, plus HCAS.
High‑Cost Area Supplement: 15% of the basic salary (minimum £4,313, maximum £5,436, pro‑rata for part‑time staff).
Permanent, Full‑time.
Working pattern: Full‑time, flexible shifts across 7 days a week, 24 hours a day.
Probationary period applies. Internal applicants are exempt from the probationary period.
