Qualified Social Worker, Hospital Discharge Team

Pay rate to £30 per hour

Contract role

We are recruiting for an experienced Social Worker to work in a Hospital Discharge team in Manchester City Council.

Job Purpose

A qualified social worker is required for the hospital discharge to assess team South. The post is based in Wythenshawe hospital, south Manchester and involves supporting patients in both Wythenshawe hospital and Trafford General with arranging safe and timely discharges back to theirown home or 24-hour care setting.
