Qualified Social Worker, Community Learning Disability Team

Pay rate £30 per hour

Contract role

Vitalis are recruiting within for a Qualified Social Worker to work in a Community Learning Disability Team in Trafford Council.

The Community Learning Disability Team is co‑located with colleagues in health. They are dedicated to assess, support plan and complete interventions for people aged 18 upward who have a diagnosed Learning Disability and connected needs (note that there is some work related to planning and preparation for adulthood – aged 14‑17).

Main tasks
  • Assessing the care and support needs of adults/carers in accordance to guiding legislation – The Human Rights Act (1998) / The Mental Capacity Act (2005) / The Autism Act (2009) / The Equality Act (2010) / The Care Act (2014) and The Down Syndrome Act (2022)
  • Interventions that relate to safeguarding adults
  • Completing capacity assessments and when required Best Interest Meetings/decisions in line with the Mental Capacity Act 2005.
  • Supporting adults who may lack capacity to consent to their accommodation and care needs and applying to the Court of Protection for authorisation of any deprivation of liberty.
  • Managing a caseload providing a social work service to people with care and support needs, which will include adults at risk of harm and neglect.
  • Undertaking administration (including keeping of appropriate up-to-date records) and management of personal workload providing statistical information as required.
  • Applications and oversight when a person’s presenting needs mean that they could be eligible for CHC (Continuing Health Care) Funding
  • The role will require evidenced registration with Social Work England
