Vitalis are recruiting for an experienced Social Worker to work in an ARCH Team in Stockport.

Responsibilities

• Work within a multi-disciplinary team (MDT) to support complex cases and hospital discharges.
• Complete Care Act assessments, mental capacity assessments, and safeguarding work.
