Job Title: Qualified Social Worker – Adults Urgent and Emergency Care Team
Location: Worksop, Nottinghamshire
Rate: £TBC per hour
Contract: Ongoing
Hours: Full-time or Part-time
Base Locations: Bassetlaw Hospital and Retford Hospital

Overview

An exciting opportunity has arisen for a Qualified Social Worker to join the Adults Urgent and Emergency Care Team in Worksop, Nottinghamshire. This role is based across Bassetlaw Hospital and Retford Hospital and supports adults to leave hospital safely and regain independence. Working within the Discharge to Assess model, you will be part of an integrated, multidisciplinary Transfer of Care Hub, contributing to timely and effective hospital discharge planning.

Purpose of the Role

To provide high-quality social work intervention within urgent and emergency care settings, supporting adults to transition safely from hospital to the community. The role focuses on assessment, planning, safeguarding, and strength-based practice to promote independence, wellbeing, and timely discharge.

Key Responsibilities

Manage a complex caseload, undertaking assessments, support planning, and reviews to promote independence and wellbeing

Complete comprehensive assessments using a range of health and social care tools, including Continuing Healthcare assessments where appropriate

Identify, coordinate, and facilitate access to Reablement and short-term support services

Promote and apply a strength-based approach, supporting individuals to assess risks and make informed decisions

Identify and utilise community, family, and natural support networks in line with the Adult Care Strategy

Monitor and review care and support provision to ensure outcomes are achieved effectively

Work collaboratively with hospital staff, local providers, and community services to improve outcomes

Provide professional advice and guidance to service users, carers, and partner professionals

Undertake safeguarding duties and specialist roles where trained and authorised

Organise and prioritise workload effectively, maintaining confidentiality and professional standards at all times
