Job Title: Qualified Social Worker – Adults Transition Team

Job Overview: An exciting opportunity has arisen for a motivated and experienced Social Worker to join the Adults Transition Team in Huyton. This full-time, ongoing locum position offers a competitive hourly rate of £32.07 and provides an excellent opportunity to make a meaningful difference in the lives of young people transitioning from children’s services to adult support.

Key Responsibilities:

  • Conduct comprehensive social care needs assessments for young people aged 14–17 to determine Care Act eligibility.
  • Collaborate closely with families and colleagues across various teams, including children’s social work, leaving care, and adult services, to ensure smooth transitions into adult care.
  • Develop, implement, and review personalised support packages, ensuring long‑term sustainability and suitability for each individual’s needs.
  • Apply the Mental Capacity Act and best interest processes, initiating Court of Protection Deprivation of Liberty (CoP DoL) processes when required.
  • Work in partnership with health colleagues in learning disabilities, mental health, and Integrated Care Board teams to deliver cohesive and coordinated care plans.
  • Participate in multi‑agency planning meetings and contribute to transitional safeguarding frameworks.

Requirements:

  • Qualified Social Worker registered with Social Work England.
  • Experience working within adult or transition social care services.
  • Strong understanding of the Care Act, Mental Capacity Act, and transition planning.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with young people, families, and multidisciplinary teams.
