Qualified Social Worker, Adults First Contact Service Pay rate, £36 per hour Contract role

Vitalis are hiring for a Qualified Social Worker to work in an Adults First Contact Team in the London Borough of Sutton. Social Workers will be responsible for Immediate needs assessments, care and support plans for adults who require social services within the London Borough of Sutton in accordance with the council’s policies and procedures related to eligibility and assessment and navigating community services using prevention and strength based approach. The post holder will be responsible for managing various case work/referrals including complex ones ensuring that wellbeing and independence is consistently promoted and reflected in carrying out all duties and responsibilities.
