Overview

Knowsley Borough Council is seeking a skilled and experienced Qualified Social Worker to join their Adult Social Care team on a short-term project. Based at the Halewood Centre, this hybrid role offers a dynamic opportunity to work with adults facing a range of physical, mental health, and social challenges.

Responsibilities

• Manage a personal caseload with autonomy and efficiency
• Be on duty twice weekly, supporting urgent and complex cases
• Conduct assessments, safeguarding investigations, and Mental Capacity Act (MCA) evaluations
• Develop support plans and commission appropriate services
• Complete Continuing Healthcare (CHC) and Decision Support Tool (DST) assessments
• Collaborate with partner agencies including Rapid Response, Reablement, housing, health, and substance misuse services
• Promote independence through short-term interventions

Qualifications & Requirements

• Recent experience in adult social work (essential)
