Responsibilities
  • Discharge Planning & Logistics.
  • Coordinating transfers to skilled nursing facilities, rehab centers, or hospice.
  • Setting up home health care, physical therapy, or medical equipment (like oxygen or hospital beds).
  • Linking patients with community programs like "Meals on Wheels" or transportation services.
  • Clinical Assessment & Intervention.
  • Evaluating a patient\'s mental health, support system, and living situation.
  • Providing immediate support for victims of domestic violence, sexual assault, or child/elder abuse (including mandatory reporting).
  • Identifying addiction issues and facilitating referrals to detox or recovery programs.
  • Navigating the bureaucracy of healthcare is a nightmare for most; social workers act as the guides.
  • Helping patients understand Medicare/Medicaid or apply for "charity care" for unpaid bills.
  • Assisting with Power of Attorney (POA) and Living Wills so the patient\'s wishes are respected.
  • Acting as a bridge between the medical team and the family when there is disagreement over the care plan.
