Overview

A Vacancy at Rotherham Doncaster and South Humber NHS Foundation Trust.

To provide appropriate physical and mental health assessment and intervention for those patients who are on an extended recovery journey with secondary mental health services. To provide a treatment, including depot administration, blood taking and monitoring service to all patients in Rotherham who are currently prescribed Clozapine and Lithium therapies.

There is an expectation that the post-holder will work collaboratively with the multi-disciplinary team, to meet the needs of patients.

Responsibilities
  • Assessment of need and risk.
  • Working in clozapine clinic.
  • Planning and delivering care, including administration of depot medication.
  • Physical health monitoring in alignment with best practice for SMI including ECGs, venepuncture and physical health checks.
  • Liaising within the multi-disciplinary care team, including carers, voluntary agencies etc.
  • To provide information as required.
  • To maintain accurate, contemporaneous, accessible records according to both NMC and Trust record keeping standards.
  • To liaise with GPs, primary care and all relevant services across the trust.
