A regional healthcare provider in Rotherham seeks a healthcare professional to conduct mental health assessments and provide care management for patients on clozapine and lithium therapies. The role involves close collaboration with a multidisciplinary team to ensure the needs of patients are met and includes responsibilities such as physical health monitoring and documentation. Ideal candidates will have experience in mental health assessments and a commitment to providing quality care.
A Vacancy at Rotherham Doncaster and South Humber NHS Foundation Trust.
To provide appropriate physical and mental health assessment and intervention for those patients who are on an extended recovery journey with secondary mental health services. To provide a treatment, including depot administration, blood taking and monitoring service to all patients in Rotherham who are currently prescribed Clozapine and Lithium therapies.
There is an expectation that the post-holder will work collaboratively with the multi-disciplinary team, to meet the needs of patients.
