Job summary

Eden Brown Synergy is seeking a Locum Social Worker to join the South and West Herts Older People & Sensory Services Team within Adult Care Services. The role supports older people and adults with sensory needs, promoting independence, wellbeing, choice and control.

The successful candidate will manage a varied caseload, undertake Care Act assessments, and develop person-centred care plans with individuals, families and professionals.

Responsibilities

• Manage a caseload of adults, primarily older people and individuals with sensory needs.
• Undertake Care Act assessments, reviews and reassessments.
• Develop person-centred support and care plans in partnership with individuals, families and relevant professionals.
• Promote independence, wellbeing and least-restrictive interventions.
• Safeguard adults at risk and respond appropriately to safeguarding concerns.
• Work collaboratively with health professionals, care providers, families and other agencies.
• Assess risk and develop appropriate risk management plans.
• Support individuals through changes in circumstances, including hospital discharge and transitions where appropriate.
• Make appropriate referrals and signpost individuals to community and voluntary services.
• Maintain accurate, timely and high-quality case records in accordance with statutory and organisational requirements.
• Work within relevant legislation, policies and professional standards.
• Contribute effectively to multidisciplinary and team meetings.

Working Arrangement: Hybrid working

Pay Rate: £32 per hour

Contract: Locum / Temporary

Driver: Full UK driving licence and access to a vehicle required

About the Role

We are currently seeking an experienced and motivated Locum Social Worker to join the South and West Herts Older People & Sensory Services Team within Adult Care Services.

The successful candidate will work as part of a multidisciplinary team supporting older people and adults with sensory needs, promoting independence, wellbeing, choice and control.

This is an excellent opportunity for a social worker with strong adult social care experience who is confident managing a varied caseload and working within a strengths-based, person-centred approach.
