About the Role/Responsibilities:

The Home First Occupational Therapy Team has recently expanded in response to the growing demand for support with hospital discharges, active recovery, and reablement packages. We are currently seeking an experienced full‑time Occupational Therapist to support this growing and busy team.

This role involves working closely with our partners across health and community services to build strong collaborative relationships that positively influence individual health and care outcomes. The OT team is a vital part of the Home First Social Care Team, operating across all areas of the county. Each team member maintains links with all three area clusters, ensuring a cohesive and integrated approach to service delivery. This means that the suitable individual will need to be able to complete visits across the county. The position is based out of Dorchester or Sherborne – the role is hybrid.

To provide information and advice to help people stay healthy, plan for their future needs, use services effectively, take responsibility for their own health and care, and maximise the impact of prevention and recovery services. To encourage and support people to look after themselves, their families and their communities, promoting choice and control for people and the use of personal budgets and direct payments where possible.

Qualifications:
  • Qualified HCPC registered Social Worker
  • Experience of undertaking assessment, support planning, and development and review of person‑centred outcomes
  • Knowledge of Legal Literacy
  • Must have own transport and be able to travel across North Dorset

Hybrid working – Monday to Friday, 37 hours. Local knowledge. Immediate start. Current end date of 31st March 2026. Subject to an Enhanced DBS and Referencing.
