A healthcare provider in England is seeking a QOF Recall & Long-Term Conditions Coordinator/General Administrator. The practice is looking for an organised, motivated and patient‑focused QOF Recall & Long-Term Conditions (LTC) Coordinator/Administrator to join our friendly GP practice team. This is a key role that supports the delivery of high‑quality patient care and ensures we meet the Quality and Outcomes Framework (QOF) and contractual requirements.
The post‑holder will take responsibility for coordinating and monitoring the recall system for long‑term condition reviews and other QOF‑related activity. You will work closely with the clinical and administrative teams to ensure patients are invited, booked, and followed up appropriately.
