Job summary

A healthcare provider in England is seeking a QOF Recall & Long-Term Conditions Coordinator/General Administrator. Responsibilities

  • Manage and maintain the practice recall system for all long-term condition reviews.
  • Generate and monitor QOF and practice performance reports.
  • Contact patients to arrange appointments.
  • Ensure that patients receive timely invitations and follow-up reminders.
  • Identify patients overdue for reviews and coordinate proactive outreach.
