Nottinghamshire Healthcare NHS Foundation Trust is seeking a Practitioner to support the Healthy Family Team in delivering the 0-19 Agenda, coordinating information flows via telephone, Chat Health and SystmOne, and ensuring accurate recording of information.

The post holder will supervise Band 4 staff, provide advice to the team, and work flexibly to meet service needs while collaborating with partner organisations to improve local child health outcomes.

Responsabilités

• Manage daily information flow via telephone, Chat Health and Parnetline.
• Signpost and provide information for parents and young people in an understandable way.
• Provide managerial supervision for Assistant Public Health Practitioners and for Band 4 team members.
• Work flexibly to meet service needs and support the Healthy Family Team in delivering the 0-19 Healthy Child and Public Health Nursing Programme.
• Collaborate with other staff, voluntary and statutory organisations to respond to community needs.
