Job summary

Westminster Lakeland, a healthcare facility in Lakeland, FL, is seeking a dedicated Physical Therapist Assistant to support our PT team. The role involves assisting with evaluations, carrying out care plans, and ensuring accurate documentation.

Qualifications

• AA degree from an APTA accredited program.
• Two years' experience in a health care setting preferred.
• Outpatient experience is required.

Responsibilities

• Carry out Care Plan as designed by the Physical Therapist (PT). Assist the PT during evaluations.
• Monitor resident's progress and communicate with PT.
• Maintain documentation in system of resident's activities, record progress and document billing activities.
• Perform Resident's Home Assessment as part of the discharge process as directed by the PT.
• Perform Resident's screening during the admission process and quarterly & annual screening to determine the need for therapy as directed by the PT.
