Job summary

Oxford Health NHS Foundation Trust is seeking a Psychiatric Nurse to join a Community Mental Health Team in Abingdon. This full-time role (37.5 hours) involves serving as the allocated key worker for a caseload of patients with severe and enduring mental illness, within a multidisciplinary team.

You will undertake bio-psycho-social assessments, provide brief interventions up to eight weeks and help develop comprehensive treatment plans.

Qualifications

  • Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management).
  • Evidence of Continuing Professional Development (CPD) portfolio, To have experience of working within a multidisciplinary team.
  • Evidence and experience of risk assessment and risk management, To have excellent English written communication skills.

Responsibilities

  • Be the allocated key worker for a caseload of patients with severe and enduring mental illness.
  • Participate in assessment, collaborative care planning and review of patient needs.
  • Undertake bio-psycho-social assessments and offer brief evidence-based interventions (up to eight weeks).
  • Work with service users and families to develop treatment plans supporting safety, well-being and community inclusion.
  • Triaging new referrals and identifying if a further assessment is required.

Our service includes three locality-based teams aligned with GP practices. You will join a dynamic multi-disciplinary team, working alongside a range of clinical professionals and closely with third sector partners to provide high-quality, holistic, person-centred care. This is a full-time role (37.5 hours, Monday-Friday, 09:00-17:00). Due to the nature of the role, the ability to travel independently across Trust sites is essential. Join us and play a key role in shaping the future of community mental health care., The role will involve being the allocated key worker (formerly known as Care Coordinator) for a caseload of patients with severe and enduring mental illness. In these instances, you will be playing an integral role as part of a multi-disciplinary team in that person's recovery, which would include the assessment, collaborative care planning and review of the person's needs. You will also be expected to undertake bio-psycho-social assessments for patients referred to the team, and this may include offering brief evidence based interventions for up to eight weeks, ensuring to work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community. This may include working closely with partner agencies.You will also be expected to complete duty, which includes triaging new referrals (we receive referrals from the Keystone Hubs, GPs, EDPS etc) and identifying if the patient requires an assessment within the AMHT or signposting to other services.
