Job description

As a Project Worker across both our Community Wellbeing Services and Housing Support services in the Southern Trust area, you’ll have the opportunity to work in two vital spaces: delivering recovery focused group programmes and providing personalised, practical and emotional support to people living in their own homes.It’s a varied, people-centred role where you’ll see the impact of your work every dayThe main function of the service is to provide the highest possible standard of care and support to the people who use our services by actively promoting and implementing Recovery by using a strengths based person centred approach.

Responsibilities
  • Develop and maintain a trusting and professional relationship with the people who use our services and act in a supportive and empathetic manner towards them
  • Contribute to and help maintain a harmonious and therapeutic environment.
  • Support the people who use our services to manage their mental health, physical health and wellbeing through the development, implementation and monitoring of individual wellbeing and safety management plans.
  • Actively encourage the involvement of the people who use our services in all aspects of decision-making, be sensitive and seek solutions to difficulties encountered by the people who use our services.
  • Deliver care and support within the context of a social inclusion and recovery approach which is designed to work with people to find and maintain hope, re-establish a positive identity, build a meaningful life and take responsibility and control.
Skills and Qualifications
  • A full current driving licence and access to a car. Consideration will be given to alternative travelling proposals in respect of applicants with a disability who cannot hold a licence. Business Insurance is required for this post
  • Be educated to QCF/NVQ Level 2, or equivalent, in a related subject e.g. Care and 6 months paid experience supporting people within a health and social care setting.OR
  • A minimum of 12 months paid experience supporting people within a health and social care setting.
  • Have proven experience of motivating, supporting and mentoring Service Users.
  • Experience of maintaining service user and agency records
  • Working knowledge of using IT systems
