Job summary

York CVS is seeking a Social Prescribing Manager to lead the Proactive Social Prescribing Service in York. You will line manage the team, drive development and monitor performance to meet KPIs.

This fixed-term role runs until 31 March 2027, involves working across locations, and requires a degree or equivalent experience in health or social care. The advertised salary is £20,617.69 for 22.5 hours per week, with pro-rata alignment for full-time equivalents. A driving licence may be helpful.

Qualifications

• Experience of line management, operational leadership, coaching and mentoring.
• Experience of leading service development, quality improvement or organisational change.
• Understanding of Social Prescribing and person-centred approaches.
• Knowledge of health inequalities, prevention and the VCSE sector.
• Ability to build effective relationships and work collaboratively across organisations and sectors.
• Strong planning and organisational skills with the ability to manage competing priorities.
• Experience of monitoring, evaluation, commissioning or service sustainability would be an advantage.
• Degree or equivalent in health, social care, community development, public health, management or a related area.

Responsibilities

• Lead day-to-day delivery and development of the Proactive Social Prescribing Service.
• Line manage and develop Proactive team members.
• Monitor service performance, outcomes, quality standards and KPIs.
• Coordinate coaching, mentoring and workforce development across Social Prescribing.
• Support recruitment, induction, retention, succession planning and staff wellbeing.
• Identify opportunities for innovation, service improvement and future growth.
• Use data and feedback to improve service quality and outcomes.
• Build relationships with Primary Care Networks, NHS colleagues and partners.
• Provide operational cover for other Social Prescribing Managers when needed.
