Job summary

Bridgwater Primary Care Network is seeking an experienced Long-term condition nurse or ACP to deliver anticipatory, personalised care for patients with LTCs and frailty across community and care home settings.

The postholder will work with MDTs to reduce deterioration, support independence and improve quality of life through holistic assessment and care coordination, with a focus on vulnerable, housebound patients.

Qualifications

• NMC registered with current practicing status in the UK.
• Non medical prescribing qualification desirable.
• Experience in a primary care/PCN environment is a plus.

Responsibilities

• Direct clinical involvement in long term condition management.
• Delivery of nursing care to patients in the practice population.
• Provide advice, support and liaison with other practice staff.
• Manage long term conditions including diabetes and respiratory diseases.
