What were looking for:
The post holder will need to be an experienced clinician who has ideally had experience working in primary care, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
With appropriate clinical assistance from the clinical hub and duty doctor, they will demonstrate safe, clinical decision-making and expert care for patients within the general practice.
The post holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
The post holder will build effective working relationships with other health care professionals.
The post holder will work within the clinical on-the-day team to deliver excellent primary health care services within the practice.

DESCRIPTION OF THEROLE:
Thepost holder is an Emergency Care Practitioner/Paramedic, who acts within theirprofessional boundaries, supporting and working alongside a team of paramedicsand clinicians in general practice. In this role they will be supported by aclinical lead who will develop, manage, and mentor them.
Thepost holder will work as part of a multi-disciplinary team in a patient facingrole. The post holder will take responsibility for areas of chronic diseasemanagement within the practice and undertake clinical assessments, diagnostics,and treatment either in the practice or at a patients residence. To providediagnostics, including normal and abnormal findings to relate them to apossible diagnosis. To use broad clinical guidelines (such as NICE), providingassistance and feedback where appropriate.
Thepost holder will provide primary support to general practice staff with regardsto accurately triage and prioritising patients, including in emergencysituations demonstrating the use of a variety of techniques to elicit thehistory of an event/illness, including past medical/drug history whileaddressing both public and social care needs of patients in the GP practice(s).
Thepost holder will provide clinical leadership as a Paramedic, seek qualityimprovement, and manage some aspects of the quality and outcomes framework toenhance services.
Thepost holder will ensure that the practice integrates with community and localservice providers to help utilise skill mix, improve patient outcomes, ensurebetter access to healthcare, and help manage workload. The role is pivotal toimproving the quality of care and operational efficiencies so requiresmotivation and passion to deliver excellent service within general practice.
Thepost holder will be required to identify any signposting, referrals, hospitaladmission or any other services identified within the care/healthcare pathway.
Toadvise and provide support to members of the public/patients in leading healthylifestyles, disease prevention and health promotion.
Tomaintain, update and monitor accurate systematic patient records of patientinteractions, clinical care provided including adverse events
Tofulfil the requirements of an experienced practitioner acting within his/herprofessional boundaries and competencies.
Toprovide care to the presenting patient in their own home or in the surgery.This will include initial history taking, clinical assessment, diagnosis,treatment and evaluation of care.
Towork autonomously but within the Practices protocols and guidelines asdirected by the lead GP clinician from the on-the-day team.
Todemonstrate safe clinical decision-making, including high levels of criticalthinking and holistic practice.
DIMENSIONS:
Thepost holder will be required take an active role to support efficient andeffective service delivery to the practice population.
To takean active role in supporting the practice to maximise achievement against localand national targets in respect of the PMS contract, QOF and Enhanced Services.
Maintainaccurate, timely and contemporaneous records within the protocols and systemsused by the Practice
Toattend any relevant Practice meetings.
Participatein education and training events
To contributeto the induction of all members of practice staff (as required) and undertakeany relevant training sessions for staff mandatory training

Towork according to Clinical Governance and support the Clinical GovernanceAgenda.
Understandthat common patient complaints may also be the catalyst for change withinpractice procedures.
Tocontribute to risk assessments, audits and Significant Events and keep informedof the importance of Infection Control.
Toactively participate in the delivery of QOF targets and others as deemed by thearrangements in general practice finance.

Tomaintain any status / registrations required to undertake the duties of therole
Toensure all CCG and Practice structured learning programmes are kept up-to-date(e.g. Mandatory training, e-Learning etc.)
