North H&F PCN serves a well-defined geographical area that is roughly contiguous with the W12 Postcode. This is an area characterised by social housing and socio-economic deprivation, with 80% of residents falling into the most deprived two IMD quintiles and about 19,000 people being within the core 20 cohort.
The needs of this population are met by nine GP practices, which have a combined list size of about 58,000 and include:
These practices are arranged in a hub and spoke configuration, with four based at a purpose-built Centre for Health and Wellbeing in the centre of the PCN geography, two to the north in Old Oak, and three to the south along Uxbridge Road. The PCN can be walked from north to south in about 15 minutes.
The PCN are investing into these roles recognising that these are needed to support them with their ongoing development. The post holder will support the PCN to continue to grow and mature so the role represents an exceptional opportunity to make a difference and establish a strong foundation for further development as part of this journey.
