An exciting opportunity has arisen for a Band 7 Primary Care Mental Health Practitioner (PCMH) for CYPS as part of the Lifespan Model which has been developed within South Tyneside.
This role will provide the post holder with the opportunity to apply and use their skills within primary care making sure that children, young people and their families and carers receive appropriate and timely assessment, while working with partner agencies.
This is a very exciting, diverse, and autonomous role, as such it will be vital to have positive working relationships with the primary and secondary care team, as well as third sector organisations.
We are looking for a Band 7 Primary Care Mental Health Practitioner who will work directly into the GP surgeries, working into the MH pathway to be a step-down support for families and young people with time limited interventions at key times in a young person\u2019s life.
The postholder should have extensive experience of working with children and young people within a mental health setting and who can work autonomously within GP practices and is also an effective team member.
