Are you passionate about transforming mental health support where people live, work, and thrive?Were looking for a dedicated Registered Mental Health Nurse to join our innovative, compassionate,and community-focused team. In this role, youll support people with mild to moderate mental healthneeds right at the heart of primary care where early support can have the greatest impact.

You will work alongside GPs, Physiotherapists, Clinical Pharmacists, Occupational Therapists, Podiatrists,Social Prescribers, and other allied professionals to offer joined-up care that puts patients first.Together, were rethinking how mental health is managed in General Practice and we want you to bepart of it.

Main duties of the job

Role Profile

As a Mental Health Nurse working at Enhanced Level Practice, you will play a central role in deliveringthe PCN's mental health offer. You will be embedded within practices, delivering early support toadults with low-to-moderate mental health needs, while maintaining strong relationships with localCommunity Mental Health Teams and VCSE partners.

You will help reduce referrals to crisis or specialist services by offering proactive, preventative, andperson-centred care. You will be expected to work at enhanced level: managing complexity, leadingservice development, and actively shaping a population-based mental health offer.

Job responsibilities

Clinical Practice

  • Deliver timely, person-centred care for adults with low to moderate mental health needsreferred from within the PCN - through GP triage, direct referral, or proactive outreach.
  • Conduct comprehensive mental health assessments and develop shared care plans withpatients.
  • Deliver brief interventions, psychoeducation, and proactive medication support (if trained).
  • Refer and signpost to IAPT, CMHTs, crisis services, and VCSE organisations as appropriate.
  • Work collaboratively with GPs, ANPs, Social Prescribers, OTs, and other team members toplan joined-up care.
  • Work autonomously with confidence to manage complexity and psychosocial need.
  • Apply holistic clinical reasoning and shared decision-making to develop meaningful careplans.
  • Apply safeguarding principles and act quickly when crises arise.
  • Actively contribute to mental health MDTs and population health discussions at PCN level.

Evidence, Research, and Development

  • Contribute to service development, quality improvement, and mental health strategyalignment.
  • Promote public mental health, prevention, and early intervention as part of populationhealth goals.
  • Identifying high-risk populations, promoting mental wellbeing, and tackling socialdeterminants of mental health (e.g. loneliness, housing, debt)
  • Act as a clinical role model within the MDT, supporting professional development andreflective practice.
  • Record activity and outcomes in line with reporting requirements.

Experience, knowledge and qualifications

  • You will be a registered Mental Health Nurse with the NMC.
  • You will be able to demonstrate proven experience supporting adults with mental healthneeds, ideally in a community or primary care setting.
  • You will have a deep understanding of common mental health conditions and evidence-basedinterventions.
  • You will have excellent communication, triage and consultation skills.
  • You will be able to work independently while contributing to a wider MDT.
  • You will have an up-to-date CPD portfolio demonstrating that you have knowledge of thePrimary Care and General Practice Nursing Career and Core Capabilities Framework andable to demonstrate your ability to operate at enhanced level practice.
  • You will have an Independent Prescribing (V300) qualification (ESSENTIAL).
  • You will have experience working within or alongside a GP practice (desirable).This role profile is not exhaustive and you may be directed to complete other tasks as required.These duties will be always be reasonable and deemed within the expectations of your position.
