Overview

Primary Care Mental Health GP Plus Practitioner – Stevenage and North Hertfordshire.

Join an outstanding Trust to develop and deliver community treatment in mental health, in line with the 10 Year Plan for Mental Health.

Responsibilities
  • Provide assessments and brief interventions for individuals and families.
  • Work alongside GPs, other primary care professionals and wider community agencies.
  • Undertake risk assessment and decide on access or re‑access to secondary mental health services.
  • Be a core member of primary care teams within a defined area, using specialist mental health knowledge.
  • Develop links and foster a culture of partnership between Primary Care Teams and HPFT Adult Mental Health Services.
Qualifications
  • Registered mental health professional (nursing, social work, OT).
  • Evidential continuous professional development.
  • Post‑registration experience in community/primary care setting.
  • Experience working with complex mental health problems and conducting clinical assessments including risk assessments.
  • Experience working consultatively with professionals and providing a range of clinical interventions to people in the designated care group.
