We are looking to recruit a highly motivated, innovative and enthusiastic Primary Care Clinical Team Lead to join our forward‑thinking, friendly and expanding Health & Justice team at HMP Downview.

By joining our Prisons Health Care Team, you will look after the health needs of patients residing in secure environments and have a real impact on promoting health and well‑being.

Responsibilities
  • Plan, deliver and evaluate clinically appropriate interventions using evidence‑based models.
  • Set up processes and protocols for effective assessment, planning, implementation and evaluation of care for all patients with a range of physical health needs, including those with mental health and/or substance misuse needs.
  • Safeguard vulnerable patients in line with Trust safeguarding policies.
  • Provide brief interventions – health education, advice and information – to patients and develop health promotion programmes in partnership with the prison.
  • Provide specialist clinical advice, training and support to nursing colleagues on patient management.
  • Apply local risk assessment and management protocols and make appropriate referrals to other services for long‑term or complex care management (e.g. detox, counselling, psychology).
  • Advise Primary Care nursing staff and GPs on medication issues and promote adherence to local medication policies and compliance with legal and professional standards.
  • Co‑ordinate multi‑disciplinary clinical practice to provide focused, integrated care and treatment programmes.
  • Lead local audits and participate in Trust audits to enhance quality of care.
Qualifications
  • NMC Registered Adult Nurse.
  • Education to degree level or appropriate experience.
  • Experience at Band 6 or equivalent in a multi‑disciplinary staff team, including line management and supervision responsibility.
  • Experience of supporting staff through change and multi‑professional collaboration, including partnership working with other agencies.
  • Experience of developing and running Nurse‑Led Clinics.
  • ENB998/SLICE or Mentorship qualification (desirable).
