Job description
Overview

Responsible for implementing an effective medicine management system within the practices, including cost‑effective prescribing, working as an Independent Prescriber and providing advice and support for both patients and colleagues. The role is pivotal to improving the quality of care and operational efficiencies in General Practice.

Responsibilities
  • Conduct comprehensive medication reviews and assessments to identify therapeutic needs and ensure safety and efficacy.
  • Prescribe medications and treatments under collaborative practice agreements, considering patient preferences, age and medical history.
  • Manage repeat prescribing reauthorisation process, reviewing requests for repeat prescriptions and medicines reaching review dates.
  • Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure medicines are obtained post discharge.
  • Provide leadership on quality improvement and clinical audit, managing aspects of the Quality and Outcomes Framework.
  • Increase quality and safety of prescribing through audit and PDSA cycles, managing practice formularies and implementing drug withdrawals and alerts.
  • Run face‑to‑face medication review clinics, including anticoagulant monitoring and normal on‑day booking clinics.
Qualifications – Essential
  • Undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council.
  • Minimum of 2 years postgraduate experience in pharmacy, demonstrated within a practice portfolio.
  • Member of the Royal Pharmaceutical Society (RPS) and working towards RPS Faculty membership.
  • May hold or be working towards a prescribing qualification and a postgraduate pharmacy qualification.
Qualifications – Desirable
  • Experience of working in a GP and holding clinics.
  • Behavioural competencies and skills for this job – demonstrating general pharmaceutical skills and knowledge in core areas, planning, managing, monitoring and reviewing general pharmaceutical care programmes.
