Job summary

Collingham Church View Surgery is seeking a Prescribing Clinical Pharmacist to lead medicines optimisation, carry out structured medication reviews and reduce risk of harm for patients.

Based in a well-established GP practice, you will collaborate with GPs, nurses and admin staff to review long-term conditions, respond to MHRA alerts and support safety with a focus on quality improvement and patient-centred care. Independent prescriber status and primary care pharmacy experience are essential.

Qualifications

  • Independent Prescriber qualification recognised by the GPhC.
  • Membership of Primary Care Pharmacy Association (PCPA).
  • Membership of the Royal Pharmaceutical Society.

Responsibilities

  • Support the practice with medicines optimisation.
  • Carry out patient-facing or telephone medication reviews.
  • Structure medication reviews with direct patient contact.
  • Management of long-term conditions.
  • Reviewing systems for safer prescribing.
  • Contributing to repeat prescriptions authorisation & reauthorisation.
  • Actioning acute prescription requests.
  • Contributing to achievement of QOF and local quality improvement schemes.

Job description

Themain aim is to improve the outcomes for patients from their medicines andreduce the risk harm. Part of this will involve improving medicines safety andmedicines management processes across the practice, such as contributing inactioning MHRA alerts and reconciling of medicines. You will carry outstructured medication reviews to optimise medication for complex patients.

Main duties of the job

Thefollowing are the core responsibilities of the Prescribing Clinical Pharmacist.There may be on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels.

a.To support the practice with all aspects of medicines optimisation

b.Patient facing or telephone medication reviews

c.Structure medication reviews with direct patient contract

d.Management of long-term conditions

e.Reviewing systems for safer prescribing

f.Contributing to repeat prescriptions authorisation and reauthorisation

g.Actioning acute prescription requests

h.Contributing to achievement of QOF and locally commissioned qualityimprovement schemes

i.Undertaking clinical audits
