Job summary

Primary Integrated Community Services Ltd is seeking an independent prescribing Clinical Pharmacist to work within General Practice, supported by a GP who will mentor and develop you. You will operate within professional boundaries while contributing to medicines optimisation alongside a team of pharmacists.

You will review patients with single or multiple conditions, make recommendations to doctors and nurses, conduct clinical medication reviews, and support care home teams to improve medicine

Qualifications

  • Masters degree in Pharmacy (MPharm)
  • Mandatory registration with General Pharmaceutical Council
  • Qualified Independent Prescriber

Responsibilities

  • See patients with single or multiple medical problems where medicine optimisation is required and review ongoing need for each medicine.
  • Undertake clinical medication reviews with patients and produce recommendations for Nurses and/or GP on prescribing and monitoring.
  • Work with care home staff to improve safety of medicines ordering and administration.

Job description

The post holder is an independent prescribing Clinical Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of Pharmacists in General Practice. In this role they will be supported by a GP who will develop, manage, support and mentor them.,

  • See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g., COPD, asthma). Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e., medicines optimisation). Make appropriate recommendations to Senior Pharmacists or GPs for medicine improvement.
  • Undertake clinical medication reviews with patients and produce recommendations for Nurses and/or GP on prescribing and monitoring.
  • Undertake clinical medication reviews with patients and produce recommendations for theNurses or GPs on prescribing and monitoring. Work with care home staff to improve safety of medicines ordering and administration.
