Job description

As an Assistant Practitioner, you'll support registered professionals in assessing, planning, and delivering high-quality care for service users. Working within a multidisciplinary team, you'll help individuals achieve their personal recovery goals while maintaining dignity, independence, and wellbeing.

Responsibilities
  • Support the delivery and evaluation of personalised care plans.
  • Work collaboratively with nurses, therapists, and other healthcare professionals.
  • Promote patient choice and participation in care decisions.
  • Maintain high clinical standards and follow relevant policies and procedures.

Qualifications
