Job summary

Whitstable Medical Practice is seeking a motivated Practice Nurse with experience in Chronic Disease Management to join our friendly, multidisciplinary team across three sites in Whitstable.

You will contribute to asthma, COPD and diabetes care, support long‑term condition management, and promote health and wellbeing through patient education and preventive care.

Qualifications

  • Registered Nurse with current NMC registration and practising qualifications.
  • Experience in chronic disease management is desirable.
  • Knowledge of EMIS or similar record systems.
  • INR, family planning, cervical cytology and travel clinic experience are a plus.

Responsibilities

  • Support chronic disease management and care for patients with long‑term conditions.
  • Lead management of asthma, COPD and diabetes in line with clinical guidelines.
  • Undertake assessments, plan and review individualised patient care plans.
  • Promote health education, vaccinations and disease prevention within the practice.

Practice Nurse with Chronic Disease Management Experience

Whitstable MedicalPractice is looking for a motivated, compassionate and clinically confidentPractice Nurse with experience in Chronic Disease Management to join ourfriendly and supportive multidisciplinary team.

This is an excitingopportunity for a nurse who enjoys developing long-term relationships with patientsand has a particular interest in the management of Asthma, COPD and/orDiabetes, alongside the wider range of services delivered within primary care.

We are dedicated to delivering high quality care to our 45,500 patients across three sites: Estuary View Medical Practice, Chestfield Medical Centre and Whitstable Medical Centre.

The successful candidate will be required to work flexibly across all three locations, as required. We welcome applicants from candidates seeking full time hours, we would also consider two part time applicants.

The successfulcandidate will be supporting the Practice's chronic disease managementprogramme, helping patients to manage their conditions effectively, improvetheir health and wellbeing, and reduce the risk of complications.

The successfulcandidate will undertake autonomous clinical practice within their professionalscope and will contribute to the delivery and development of chronic diseaseservices across the Practice.

Keyresponsibilities will include:

Taking a lead role in the management and review of patients with Asthma,COPD and/or Diabetes, in line with current evidence-based guidance.

  • Undertaking comprehensive clinical assessments and developingindividualised management and treatment plans.
  • Monitoring disease control, treatment effectiveness and patientoutcomes.
  • Supporting patients to understand and manage their long-term conditions,encouraging self-management and healthier lifestyle choices.
  • Identifying patients at increased risk of complications or deteriorationand taking appropriate action.
  • Recognising when referral to GPs, specialist teams or secondary care isrequired and ensuring appropriate onward referral.
  • Supporting the identification and proactive management of patients atrisk of developing long-term conditions.
  • Providing health promotion and preventative healthcare advice.
  • Working collaboratively with GPs, Advanced Practitioners, Pharmacists,Healthcare Assistants and the wider Practice team.
  • Requesting and reviewing pathology investigations where appropriate and acting on results within scope of practice.
  • Maintaining accurate, contemporaneous and confidential clinical records in accordance with professional and legislative requirements.
  • Supporting patients with complex and long-term health needs and liaising with community and specialist services where appropriate.
