Practice Nurse with Chronic Disease Management

We are looking to recruit a Practice Nurse with CDM experience who is highly motivated, proactive and positive to join our clinical team. You will need to be competent in all the usual treatment room duties including wound care, compression bandage, cervical smears, baby and travel immunisations.

Main duties of the job

We are looking for an experienced primary care nurse that is able to hit the ground running.

It's a busy environment, so you will need to be able to work under your own initiative as well as part of a successful team. Additionally you will need to be flexible and amenable to the changing demands of primary care.

About us

St Mary Street is a well established Practice located in the semi rural town of Thornbury near to the Severn Estuary. We are a dedicated team of 30 people (directly employed) who are supported by PCN employed colleagues including a Pharmacy team, Physiotherapists, a Frailty team and Social Prescribers.

We are a friendly, successful 4 Partner Practice committed to providing high quality patient health and care services to our 9000 patients and growing. Our ethos is built upon some core values including safe, accessible and continuous care for each of our patients.

We are also a GP training practice with 3 of our 4 Partners registered as trainers. Therefore, we commonly have learners with us at any given time. In addition, we are supported by three Salaried GPs, an Advanced Nurse Practitioner, a highly successful nursing team and a dedicated and supportive administrative team.

We are a social bunch and fair well in the annual GP Patient Survey. Our staff turnover is low and the team supports and looks out for each other especially as patient demand on our services reaches an all time high.

Job responsibilities
  • Implement and embed health promotion and well‑being programmes
  • Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects on patients health
  • Provide routine nursing care to patients as required in accordance with clinical‑based evidence, NICE and the NSF
  • Provide wound care (ulcer/Doppler etc.) to patients
  • Provide immunisations (childhood, seasonal, etc)
  • Provide cervical cytology screening
  • Request and process pathology results
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure accurate use of SNOMED codes to support QOF and LES/DES targets
  • Chaperone patients where necessary
  • Assist GPs with minor surgery when required
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Implement vaccination programmes for adults and children
  • Support patients in the use of their prescribed medicines or over‑the‑counter medicines (within own scope of practice)
  • liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Deliver opportunistic health promotion where appropriate.
