This is a permanent, part‑time position offering the chance to work with a friendly, supportive multi‑disciplinary team and deliver high-quality primary care services to a growing rural patient population.

Registered Nurse (NMC)
Experience in primary care is desirable, but training will be provided for the right candidate.
Strong communication and team‑working skills.
Main duties of the job
- Provide general nursing services including chronic disease management, immunisations, wound care, and health promotion.
- Work collaboratively with GPs, nurse practitioners, and HCAs to provide holistic patient care.
- Manage QOF and long‑term condition targets.

Job Responsibilities
Key Responsibilities
Clinical practice
- Provide assessment, screening treatment, services and health education advice.
- Undertake the collection of pathological specimens including intravenous blood samples, swabs, smears, etc.
- Assist with the organisation and co‑organisation of the provision of nursing services for the practice.
- Provide general nursing treatment to patients in participation with general practitioners using agreed protocols.
- Assess, plan, implement and evaluate individual treatment plans for patients with a known long‑term condition to include health promotion and well‑being.
- Be competent and confident in managing people with long‑term health conditions.
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long‑term condition as appropriate.
- Work with other health care professionals to diagnose, monitor, manage and treat long‑term conditions, including non‑drug‑based treatment methods using a management plan, and in line with national and local policies and practice needs.
- Work with patients in order to support compliance with and adherence to prescribed treatments.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self‑care.
- Support and manage health needs of women presenting for family‑planning and cervical cytology consultations.
- Implement and participate competently in vaccination and immunisation programmes for both adults and children.
- Advise, support and administer vaccinations competently where appropriate for patient travelling abroad.
- Promote and deliver evidence‑based care for patients presenting with aural conditions.
- Assess and treat routine wounds including trauma.
- Assessing to support good wound management in complex situations (e.g. use of Doppler technique).
- Assisting the general practitioner and/or undertaking some simple procedures.
- Maintain the stock.
- Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems).
- Act as a chaperone when required following the practice chaperone policy.

Experience
- Pleasant, polite and articulate.
- Experience of working autonomously.
- Self audit and reflection.
- Organised and efficient in record keeping.
- Time management being able to prioritise work and work under pressure.
- Ability to record accurate read coded data entries.
- Experience in General Practice or community nursing.
- Experience of SystmOne clinical system.
- Experience of managing Long Term Conditions.
- Experience of quality initiatives i.e QOF.
Qualifications
- Fully qualified RGN or equivalent degree with NMC registration.
- Enhanced Disclosure & Barring Service check Clear.
- Evidence of continued professional development.
- Cervical Smear Taker.
- Qualifications/certificates in Chronic disease Management eg Respiratory.
- Nurse Prescriber.
