Practice Nurse - Park Royal Medical Practice

An exciting opportunity has arisen for an experienced practice nurse to join a well established and thriving GP Practice in North West London on a part time basis (3 days a week)

Main duties of the job

The practice nurse will work as part of an excellentadministrative and multidisciplinary clinical team to provide high qualitynursing care to patents. They will play a key role in patient education andsupporting self-care.

Providing and evaluating a range of healthinterventions to ensure that patients health and social care needs are met.

Health and well-being promotion plusindividual person-centred plans for patients with/at risk of a long-termcondition.

Managing and treating long-term conditions inline with national and local policies and practice needs.

Reviewing medication for therapeuticeffectiveness, in accordance with evidence-based practice and national/practiceprotocols and work with patients to support adherence to prescribed treatments.

Prioritising health problems, interveningappropriately, to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care; Supporting andmanaging health needs of women presenting for family planning and cervicalcytology consultations.

Implementing and participating in vaccinationand immunisation programmes for both adults and children including patientstravelling abroad

Meeting the needs of patients presenting foropportunistic wound care.

Hypertension review and management; RunningAsthma, COPD, Diabetes clinics and reviews.

New patient and health checks.

Park Royal Medical Practice is an APMS general practice run by HarnessCare Ltd. It currently cares for 9,000patients living predominantly in Brent and Ealing Boroughs. The practice is part of Harness South PrimaryCare Network (PCN) and Harness Care Ltd provides support to both Harness Southand Harness North PCNs. The combinedpopulations of the two PCNs is 150,000 patients providing a range ofopportunities for career progression and working in innovative new ways ofdelivering care. The practice is a keyhub site for the local area and the development of neighbourhood working andIntegrated Neighbourhood Teams.

Experience
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care and recording spirometry
  • Knowledge and application of supporting long-term conditions.
  • Insight and understanding of current policies and practices in nursing and the NHS.
Qualifications
  • A relevant nursing degree and at least 2 years post qualification experience in a General Practice setting
  • Current NMC registration with an active PIN
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications
