Practice Nurse Cantilupe Surgery Hereford

Cantilupe and Hampton Dene Surgeries are looking for an enthusiastic part time (25-30 hours) Practice Nurse to join our established nursing team.

We are a training practice and are committed to delivering high quality care to our local community.

Main duties of the role

Main duties of the role are long term condition monitoring.

We would welcome candidates with additional qualifications in diabetes in particular. However, a good ‘all‑rounder’ with a broad range of chronic disease skills, would fit in well in our team and for the right candidate, additional training will be considered.

A prescriber would be very welcome but not essential for the role.

Applicants need to be an established practice nurse in a GP surgery, and have broad treatment room skills such as cervical cytology, travel imms, childhood imms, blood pressure monitoring and management, phlebotomy, dopplers, dressings including compression, suture removal… etc.

Job responsibilities
  • Assist with the assessment, planning and implementation of agreed care in relation to the management of long-term conditions, including annual and follow up health reviews specifically for this post. This includes:
    • Diabetes
    • Asthma
    • COPD
    • Coronary heart disease
    • Heart Failure
    • Family planning
    • QOF areas, learning disability checks and mental health reviews
  • Advise patients on general healthcare with referral to GP as necessary.
  • Perform a holistic assessment of patients attending for a range of Practice nursing duties, including (but not exclusive):
    • Cervical screening
    • Wound care / ulcer care / Doppler including compressions bandaging
    • Removal of sutures
    • Well man/woman checks
    • Travel advice and immunisation
    • Contraception and women’s health advice, including HRT
    • Venepuncture
    • Routine immunisations / Childhood immunisations
    • Chaperoning and assisting patients where appropriate who are being examined by another clinician
    • Assisting with minor surgery and coil fittings, as requested
    • Requesting pathology tests, for example urine culture, swabs

Cantilupe and Hampton Dene Surgeries are based in Hereford city and cover 12,600 patients. We have a team of 3 partners, 4 salaried GPs, 4 Practice Nurses, 3 HCAs and a large admin and reception support team.

We were rated Good by CQC in 2024. The main surgery is based in Hereford city with the branch around 2 miles away. Both sites are purpose built and modern.

Person Specification
  • Good standard of education – GCSE top 4 grades or A level A‑C
  • Registered Nurse with current NMC registration
  • CPD development and training evidence
  • Experience of working in General Practice and part of MDT team
  • At least one specialist long term condition area – preferably diabetes
  • Qualification as Practice Nurse
  • Qualification in chronic disease areas – more than one inc diabetes ideally
  • Good IT skills including previous use of EMIS clinical system
