The Practice Nursewill deliver high-quality, patient-centred care within the Willow Tree Family Doctors practice

Main duties of the job

Diagnosis, examination and treatment of patients, preventative care, screening and patient education.

About us

Our PCN consists of 10 GP practices, caring for a population of over 75,000 patients and Willow Tree Family Doctors serves a population of up 15,000 patients.

We work together with a range of local providers, including acute trusts, community services, social care, voluntary sector and patient participation groups, to offer more personalised, co-ordinated health and social care to our local population.

We currently employ a large team including:

  • Management and Administration Support
  • Clinical Pharmacists and Pharmacy Technicians
  • Social Prescribing Link Workers
  • First Contact Physiotherapists
Job responsibilities
  • Deliver and evaluate a wide range of health interventions to meet patients health and social care needs.
  • Promote health and well-being through individualised, person-centred care plans, especially for patients with or at risk of long-term conditions.
  • Manage and treat long-term conditions (e.g., asthma, COPD, diabetes, hypertension) in line with national and local policies, including running dedicated clinics and reviews.
  • Conduct NHS health checks for new patient registrations and provide general and specific health screenings and assessments.
  • Review medication for therapeutic effectiveness using evidence-based practice and national protocols, supporting patient adherence to treatments.
  • Triage and prioritise health problems, intervening appropriately in complex, urgent, or emergency situations, including initiating emergency care.
  • Support and manage womens health needs, including family planning and cervical cytology consultations.
  • Implement and participate in adult and child vaccination and immunisation programmes, including travel vaccinations.
  • Provide wound care and perform appropriate tests such as blood samples, swabs, smears, ECGs and 24-hour blood pressure monitoring.
  • Advise and educate patients on general health care, maintaining a collaborative, supportive and holistic approach at all times.
  • Participate in patient education sessions, local health campaigns and collaborate with patient participation groups to improve health outcomes.
Person Specification
Experience
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressings and wound care
  • Experience of working in General Practice using EMIS clinical system
  • Experience with specific patient groups (e.g. elderly, children, mental health)
  • Specialist training (e.g. diabetes, asthma, travel health)
  • Experience working as part of a multidisciplinary team
  • Experience in triage

Qualifications
  • A relevant nursing degree and post qualification experience in a General Practice setting
