Job Summary

Thank you for your interest in our vacancy for a Practice Nurse. We are looking for a committed and enthusiastic person to join our innovative multi‑disciplinary team at York’s newest purpose‑built primary care facility.

The successful candidate will be a key part of our two‑partner practice and will work alongside a growing team, delivering health care to people who live and work in York, as well as staff and students at the University of York.

Unity Health currently operates two surgeries to the south and east of York, close to excellent local amenities. We are a consistently high QOF achiever and a proud member of the Humber & North Yorkshire ICB and the Student Health Association (SHA).

Main Duties
  • Accountable for the delivery of efficient, quality and evidence‑based clinical care in accordance with professional standards, guidelines and code of conduct (Safeguarding, Nursing and Midwifery Council, NICE, Local Authority Guidelines).
  • Chronic disease management (Asthma, vascular disease including IHD/Stroke/PVD, COPD, diabetes, hypertension).
  • Undertake clinical assessment and treatment of patients attending the practice.
  • Function dynamically and adjust to the changing needs of the client group.
  • Family planning including cervical cytology screening.
  • Assist in the delivery of specific campaigns (Flu/Shingles).
  • Delivery of routine childhood vaccinations.
  • Maintain stock of drugs and equipment; advise on equipment failure/maintenance.
  • Contribute to QOF data collation as required.
  • Record all patient contact on the practice medical record system in a timely manner.
  • Conduct new patient health checks.
  • Wound management.
  • Spirometry.
  • Perform monitoring of patients for whom we provide long‑term contraception and hypertensives.
  • Perform urine dip tests.
  • Review and amend nurse protocols as required.
