We are looking for an enthusiastic, experienced practice nurse to join our friendly and supportive GP practice. We are a closely knit team that takes pride in patient care. We are a training practice and we actively support our staff to develop them.

The Practice nurse is vital in delivering high quality care to our patients. We are a large practice of approximately 12,600 patients.

Main duties of the job

The Practice nurse will be vital in Chronic Disease management. Knowledge of the GMSContract, e.g. the Quality and Outcomes Framework, is a vital element of the post. The Practice Nurse will ensure care for patients is delivered under agreed guidelines (PGDs and PSDs).

Tettenhall Medical Practice is a large GP practice in leafy suburb of West of Wolverhampton serving around 12,600 patients. The practice is based in an NHS Estate with a friendly and supportive team. The workload in general practice is continuously evolving and we are in the process of re-structuring to strengthen our clinical team to help cope with these changing dynamics and to continue providing a great service to our patients.

We are looking for applicants with initiative, enthusiasm and a pleasant personality with leadership skills who can manage different tasks to fill this key position within our practice.

Job responsibilities
  1. To act as the Infection Prevention Control (IPC) lead and to work to the IPC policy, providing audits, training, support and evidence of compliance to management. Following that, produce the IPC annual statement.
  2. The Lead Nurse will be CQC lead for the nursing team ensuring services provided are effective, safe, responsive, caring and well‑led by using up to date policies and protocols.
  3. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence based practice.
  4. To develop, implement and embed health promotion and wellbeing programmes.
  5. Management of chronic diseases which includes asthma, hypertension, COPD, CVA‑TIA, CHD, and CHD risk assessment and management. Initiates the process of diagnosis with patients suspected to have chronic disease.
  6. To co‑ordinate the provision of travel vaccination services.
  7. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
  8. To maintain accurate clinical records in conjunction with extant legislation.
  9. To ensure snomed codes are used effectively by all members of the nursing team.
  10. To ensure compliance with the practice chaperone policy.
  11. To support the team in dealing with clinical emergencies.
  12. To recognize, assess and refer patients presenting with mental health needs.
  13. To lead and implement vaccination program for adults and children, working with the wider team to increase uptake.
  14. Ensure safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.
  15. Participate in the review of significant and near‑miss events, applying a structured approach.
