Main duties of the job

The successful candidate will provide a wide range of primary care nursing services, including:

Chronic disease management (e.g., asthma, COPD, hypertension, diabetes)

Childhood and adult immunisations

Cervical cytology

Wound care, dressings and suture removal

Health promotion and patient education

Supporting patients with long-term conditions

Supporting the Nursing Clinical Lead with clinical audits, data collection and quality improvement activities

Person Specification
Qualifications
  • Registered Nurse RN with valid NMC registration.
  • Evidence of ongoing CPD and revalidation.
  • Formal training in immunisations and vaccinations adult & childhood.
  • Cervical cytology training and accreditation or willingness to complete.
  • Diploma Degree in Practice Nursing or equivalent.
  • Qualifications in chronic disease management e.g., asthma, COPD, diabetes). evidence of experience
Experience
  • Experience working as a nurse in primary care community or an acute setting
  • Competence in immunisations wound care cervical cytology and basic long-term condition reviews
  • Ability to manage own clinics independently
  • Experience using clinical systems and maintaining accurate clinical records.
  • Experience working in a GP practice
  • Confidence in chronic disease management e.g hypertension asthma COPD diabetes
  • Experience with QOF NHS screening programmes and health promotion
  • Experience supporting quality improvement or practice audits
