The role involves delivering high-quality nursing care to a patient population of approximately 12,000, managing chronic diseases, and providing immunizations. We are looking for an enthusiastic and motivated Practice Nurse. The ideal candidate should have primary care experience, however, the practice would consider providing training and mentorship for the right candidate.
Responsible for the delivery of high-quality practice nursing care to our patient population of circa 12,000. The following provides a brief outline of the role.
Working autonomously, the Practice Nurse will be responsible for a number of clinical areas, to include public health, infection prevention and chronic disease management;-

We are an innovative team, keento try out new ways of working and have recently adapted how we work to makeroles as clinicians more sustainable and hopefully more enjoyable. Thisincludes the utilisation of the Total Triage model (AccuRx digital triage) through our in-houseDuty Hub.
The post holder will be an integral part of the nursing team, as well as supporting the wider general practice team. We are continuously striving todevelopour services and skill mix and as such are a happy team, who supporteach other.
