Job description

We are looking for an enthusiastic and motivated Practice Nurse. The ideal candidate should have primary care experience, however, the practice would consider providing training and mentorship for the right candidate. Responsible for the delivery of high-quality practice nursing care to our patient population of circa 12,000. The following provides a brief outline of the role. Working autonomously, the Practice Nurse will be responsible for a number of clinical areas, to include public health, infection prevention and chronic disease management;

• Long Term condition management (hypertension, diabetes, asthma COPD)
• Cytology
• Child Immunisations
• Vaccinations
• Swabs, specimens, bloods, blood pressures
• Keeping accurate and details health records on clinical system
• A key role in QOF achievement
• Involvement in the MDT of the surgery
• Telephone consultations and advice
• Discussion of blood results with patients.
• The post holder will be an integral part of the nursing team, as well as supporting the wider general practice team.
