Job description

We are seeking a motivated, compassionate, and skilled Practice Nurseto join our dedicated Asylum Seeker Health Service, supporting people seeking sanctuary to access high-quality, person-centred healthcare.

Working as part of a multidisciplinary team, you will provide holistic nursing care to individuals and families who have recently arrived in the UK, many of whom have complex physical, psychological, and social health needs. You will play a key role in supporting their initial health assessment, vaccination programmes, health promotion, and management of long-term conditions, while helping patients to navigate the healthcare system with dignity and respect.

Main duties of the job
  • To work as a member of the Practice Nursing Team and be responsible for acting within their professional boundaries to provide care for the presenting patient including screening, clinical assessment, diagnosis, treatment and follow up of care; demonstrating safe clinical decision making and expert patient care.
  • To be involved in all areas of Chronic Disease Management (Asthma, Diabetes, COPD etc.).
  • The Practice Nurse will work collaboratively with the clinical team which will include GPs and other health care professionals., ambitious for our shareholders and so we continue to work hard, both with providers and commissioners, to find new ways of maximising positive patient outcomes and experiences.
Primary Care Sheffield (PCS)

Our shareholders are 79 GP practices in Sheffield covering over half a million patients. As well as providing a unified voice for general practice in the city, we play an active role both within our Accountable Care Partnership (ACP) and the wider South Yorkshire and Bassetlaw Integrated Care System (ICS).

General practice working at scale through PCS enables us to:

  • Improve the quality of GP referrals into secondary care with the provision of education, support and referral guidance to practices across seven outpatient specialities.
  • Provide evening and weekend appointments, 52 weeks-a-year, with GPs, nurses, physiotherapists and phlebotomists at six sites around the city.
  • Deliver core general practice through the Clover Group of seven GP practices.
  • Operate 24hr ECG and vasectomy clinics and the NHS Healthchecks programme for the city.
  • Provide governance and support to practices to enable them to work together in neighbourhoods.
